The 2003-2004 valve covers look very similar cosmetically to the 2005-2006 valve covers. The difference is in the pcv valve connection area and the pcv breather system connection point. 03-04 use a plastic hose with rubber ends for pcv system. 05+ use a cam-lock system to interconnect pcv system components.
The 1992-2002 drivers side valve cover will not work on a 2003-2009 crown victoria due to the air intake zip tube passing over the area that the oil fill neck is located
The 2003-2009 passenger valve covers will not work on a 1998-2002 crown vic due to the fuel lines being located where the oil fill neck is located

Notes:

The 2003+ pcv valves contain a one-use locking system. According to the ford service manual, "CAUTION: A new PCV valve must be installed if removed. Upon removal, the plastic retaining ears of the PCV valve are sheared." You can seperate the pcv valve from the hose assembly, but the pcv valve cannot be removed from the valve cover without damage.
There are a few different designs of heated pcv valve systems avaliable on ford vehicles. One style uses a metal tube which has hot engine coolant flowing through it wrapped around the pcv valve, but this was never used in the crown vic. Another uses a custom designed intake manifold to heat the pcv area with engine coolant, but this again was never used in the crownvic. The setup that was actually implemented in 2005+ ford crown victorias with the heated pcv system uses an electric heater instead.

Generic rubber vacuum hose can be substituted for the pcv valve line assembly, but it will have a rather short life with heated oil gases flowing through it. The factory OE setup has a very short length of rubber tube at each end with a molded plastic tube in between.

United States Patent 6390080 - Intake manifold with a heated PCV passage
In very cold climate conditions, there can be a freeze up in the connection of the PCV hose with the intake manifold. To alleviate possible conditions of freeze up, some PCV systems have a fitting that is heated by engine coolant at the point of intersection between the PCV hose and the manifold.

PCV System Monitor - 2003 MY OBD System Operation Summary for Gasoline Engines
Ford plans to comply with the PCV monitoring requirements by modifying the current PCV system design. The PCV valve will be installed into the rocker cover using a quarter-turn cam-lock design to prevent accidental disconnection. High retention force molded plastic lines will be used from the PCV valve to the intake manifold. The diameter of the lines and the intake manifold entry fitting will be increased so that inadvertent disconnection of the lines after a vehicle is serviced will cause either an immediate engine stall or will not allow the engine to be restarted. Some vehicles will incorporate such designs beginning in the 2001 MY. In the event that the vehicle does not stall if the line between the intake manifold and PCV valve is inadvertently disconnected, the vehicle will have a large vacuum leak that will cause the vehicle to run lean at idle. This will illuminate the MIL after two consecutive driving cycles and will store one or more of the following codes: Lack of O2 sensor switches, Bank1 (P1131 or P2195), Lack of O2 sensor switches Bank 2 (P1151 or P2197), Fuel System Lean, Bank1 (P0171), Fuel System Lean, Bank 2 (P0174)
